YOUR Health Insurance
As a courtesy to you, we will file for benefits on your behalf with
your insurance carrier. If you wish us to file for you, it will be necessary for
you to supply us with complete and accurate information.
All charges for medical treatment are the full responsibility of the
patient and are due in full 45 days from the date of service. This will
allow ample time for reimbursement from your insurance carrier.
For your convenience, we are pleased to accept Visa, Discover, American
Express and MasterCard.
Because of continuing changes in health care regulations and the increased
cost in providing medical care, we have implemented this policy in order to
continue to provide you with the very best in medical treatment and care.
If your insurance has changed Please update our records.
Remember if you receive services form our doctors or or office you are
responsible for payment.

Common Questions:
- How do I find out if the doctors at ARFP are on my Insurance plan?
- The quickest and most reliable way is to call the customer service
number listed on your insurance ID card. You can also check your insurance
plan web site.
- Do you accept Worker's Comp insurance?
- Yes, if we have arrangements with your employer.
- What information is needed at the time of a worker's comp type visit?
- Claim number supplied by your supervisor.
- Insurance claim address.
- Your worker's comp contract number.
- The date of the injury.
- Your supervisors name and phone number.
- A photo ID such as a Drivers License.
- Does ARFP accept Auto Insurance for automobile accident injuries?
- Yes, and we will need a claim number. We will also need a photo ID, the
insurance company claims address, date of the accident and the Adjuster's
name and phone number.
- My children have AR Kids First, can they see the doctors at ARFP?
- You must contact DHS or AR Kids First before making a new patient
appointment.
- Does ARFP accept MEDICAID?
- If MEDICAID is your only insurance you must contact DHS to request a
physician as your PCP.
- If MEDICAID is secondary to MEDICARE then you do not need to contact DHS
before making a new patient appointment. However our doctors may not be
accepting new MEDICARE, please call to check.
- What is a PCP?
- A Primary Care Physician.
- I have a different doctor listed on my insurance card as my PCP, can I
still make an appointment to see the doctor at ARFP?
- Yes, but most likely your insurance will not cover the visit and you will
be required to pay for the charges.
- I made an appointment for a Yearly Physical Exam and now my insurance
tells me they wont pay for it, can ARFP 'resubmit' the claim with one of my
other diagnoses?
- It is your responsibility to know what services your insurance will
cover. If you requested a Yearly Physical Exam and the doctor preformed it
for you then you will be responsible to pay for it.
